Case report of spontaneous sino-cutaneous fistula in chronic rhinosinusitis

Abstract Background Chronic rhinosinusitis (CRS) is a common disease characterized by inflammation of the paranasal sinuses, with symptoms such as nasal blockage, facial pain, hyposmia, and headache. While these are often treated cold, can lead to serious complications if left untreated. We present an unusual case CRS in which patient presented ophthalmology department persistent pus discharge from upper eyelid for 6 months, despite being multiple courses antibiotics undergoing incision drainage procedures. Case presentation A 69-year-old male 6-month history eyelid, oral three He also complained yellowish bilateral blockage past year but did not seek medical attention symptoms. Physical examination revealed widening nasomaxillary groove. Nasoendoscopic showed grade 3 polyps mucopus secretion. CT scan sinuses that odd was due chronic inflammatory process had led osteitis hyperostosis. This underwent endoscopic sinus surgery (ESS) frontal trephination fat obliteration sinus. The postoperative histopathological report confirmed diagnosis benign polyp acute on inflammation. asymptomatic at 3-month follow-up post-operation. Conclusion Our highlights importance considering possible underlying cause presentations, even when may appear unrelated. Early treatment prevent improve outcomes. leading hyperostosis complicates clinical presentation, distorts usual anatomical structures, complicate operation. highly recommended before any surgery. At times, ESS be sufficient eradicate disease. In cases, external approach necessary or without cavity.
